New Patient Registration
If you wish to register with the practice please visit us and talk to our receptionists. It would be a great help if you could bring your up to date medical card with you. If you cannot find it simply see our receptionist and ensure you know the following details:
- Full name, including previous names
- Date and place of birth
- Last address
- New address and postcode
- Name and address of your previous doctor
- NHS number if known
All new patients are required to complete a confidential questionnaire about their medical history. This will help the doctors and nurses understand your health needs. Registration forms and questionnaires are available from reception. Once these forms have been completed you will be offered a new patient consultation.
